Summary

INTRODUCTION: Uremic patients without primary pulmonary disesae immediatly before regular hemodialysis (HD) show mild restrictive changes in pulmonary function.
Forced vital capacity and FEF25-75% incease after hemodilysis DLCO and KCO acute decrease after HD becouse pulmonary capillar volume are lowered.
One transversal study shows significant lowered values of DLCO uremic patients treated with chronic HD for a period more than 5 year in comparation with values of DLCO
uremic patient treated with same procedure less than 1 year.
It was unclear how DLCO and his membrant and vascular components changes in dependent of total duration of treatment with chronic HD in the same patient .,what we investigated with prospective one year following of same patients.
PATIENTS AND METHODS: DLCO is mesured within the group of 28patients ( 8 females and 20 males) on chronic hemodialysis.
The average duration of hemodialysis was 6.39± 5.02 years.
In 13 patients values of total lung capacity (TLC), inspitatory vital capacity (IVC), KCO and DLCO are evaluated twice a year.

RESULTS:
1.
Measured values of diffusing parameters in chronic uremic patient treated with HD (DLCO, KCO) average are 25% lowered than antropometric espected values.
Spirometrical parametrers are average decreased 15-20%.

Transversal study on 28 patients didn`t prove connection between total duration of treatment with chronic HD and parameters of pulmonary function, but one year following
DLCO, KCO and spirometric parameters in 13 patients show statististically significant decreasing values of diffusing coefficient (KCO).Decreasing of DLCO (average 8.62) and
spirometric parameters are sugestible. too.

CONCLUSION:
In chronic uremic patients on the program of HD exist decreasing diffusing pulmonary parameters ,same as, in little values, ventilating parameters in comparation with
anthropometric expected values.
Prospective following pulmonary function in patients with prolonged total duration of HD we proved significant lowering of diffusing coeffitient. Decreasing of DLCO and
spirometric parameters are sugestible, too.
Decreasing of KCO and DLCO in the same patients trought a year is more consequence of impairment DM than change of Vc.
