SUMMARY

Expression of alpha-smooth muscle actin (ASMA) in physiological conditions is limited to wall of blood vessels, when assessed by immunohistochemistry, while in glomeruli and interstitium it is weak or absent.
In glomerulonephritis (GN) increased expression of ASMA can be found in glomeruli and interstitium. Aim of this study was to establish expression of ASMA in glomeruli and interstitium of patients with GN and to analyze its correlation with actual kidney function.
Kidney tissues and clinical data were analyzed in 168 patients, both children and adults, which underwent kidney biopsy at the Clinical hospital centre Split from January 1994 to April 2007.
Glomerulonephritis was histopathologically diagnosed at 142 patients from this cohort -in 60 children (age 1-17, median 9 years) and 82 adult patients (age 18-73, median 45 years).
For these patients clinical data were collected and tissue stained with indirect imunohistohemical method, using mouse anti-ASMA antibody. Computer-assisted morphometric analysis of stained tissue was done, and surface of glomeruli and interstititum with ASMA expression was calculated.
After that, percentage of expression of ASMA in glomeruli and interstitium was calculated for each patient. For assessment of possible correlation between expression of glomerular and
interstitial ASMA and values of blood pressure, serum creatinine, creatinine clearance and 24hour proteinuria detailed statistical analysis of data was performed.
Results showed that expression of ASMA in glomeruli and interstitium between proliferative and nonproliferative GN is not significantly different (p=0,284 and p=0.446).
In children and adult patients greater expression of interstitial ASMA was linked with increase of serum creatinine (p=0,034 and p=0,0009), as well as decrease of creatinine clearance (p=0,0152 and p=0,0007).
Increased expression of ASMA in glomeruli was accompanied with normal or decreased values of serum creatinine in adults (p=0,0016) and increase of creatinine clearance in children
(p<0,001). Significantly higher interstitial expression of ASMA was found in children with increased blood pressure (p=0,0014). There was no statistical significant correlation between
expression of ASMA and values of 24-hour proteinuria (p=0,29).
Expression of ASMA in interstititum is associated with decreased kidney function -higher values of serum creatinine and decrease of creatinine clearance. Expression of ASMA in glomeruli is associated with normal or even increased kidney filtration because in adults it is more often found with referent values of serum creatinine, and in children with referent values of creatinine clearance.
Children with increased blood pressure have greater interstitial ASMA expression.
Expression of ASMA has no influence on values of 24-hour proteinuria. In GN expression of ASMA points to activation of compensatory mechanisms in the sense of hyperfiltration, and to
initial irreversible interstitial fibrosis, that is accompanied by reduction of kidney function.
